We have provided a list of our common formulations on this website for your convenience. However, we are also able to compound other non-sterile formulations tailored to your specific needs. To request a custom formulation, please contact our pharmacy directly or place your order by including the details in the "Notes" section of your e-prescription or in the "Custom Formulations" section of our prescription form. Our team is here to assist you in creating personalized solutions for your patients.

To streamline the process of ordering custom formulations from compounding pharmacies, follow these steps:

  • 1.Prescription Submission

    ● Write a detailed prescription specifying the active ingredients, concentrations, and dosage form required.
    ● Include patient-specific information and any necessary modifications.

  • 2.Choose a Submission Method

    Select the most convenient method to submit your prescription:
    ● E-prescribe: Use the pharmacy's unique identifier (NCPDP ID # 566 2449)
    ● Fax: Send the prescription to the pharmacy's dedicated fax number (510) 573-0096
    ● Phone: Call the pharmacy directly to speak with a pharmacist (510-573-0064)

  • 3.Consultation
